ANSWER

c. Infected bronchogenic cyst. There is a well defined rounded soft tissue density within the right mid zone with evidence of internal gas (refer to axial CT image below);

The patient had a longstanding bronchogenic cyst which has subsequently become infected. Bronchogenic cysts are uncommon lesions that result from abnormal budding of the ventral foregut. They are usually located in the hilar or mediastinal region are cystic structures lined be respiratory epithelium. Other potential complications of bronchogenic cysts include local compression on surrounding structures (e.g. oesophagus, blood vessels and airways) as well as internal haemorrhage.

Se observa en la radiografía de tórax en proyección posteroanterior; opacidad de bordes regulares la cual es heterogénea con dos áreas lúcida en su porción superior, de localización en la región hiliar del hemitorax derecho que hace signo de la silueta con la vena cava superior y bronquio principal derecho por lo que su origen es en mediastino medio. Seems to be rathet homogenous well shaped shadow with exavation of upper part. Most probably infected bronchogenic cyst (C), but answer A should be also reviewed.
CT with contrast should be recommended.
